PLEASE REMEMBER THAT:
-Please come to your appointment alone and not accompanied. We do not admit children or any other person that may accompany you.

–Face Masks are still required at Medical Facilities
–Government Issued Photo Identification Is Required ( We do not accept Temporary ID Paper ID’s or Copies or pictures of Identification) For verification , we need the actual non-interim Identification Credentials.

Please remember that Original Government issued Photo Identification will be needed at the time of your service. We cannot accept Copies or Pictures of your IDENTIFICATION. Temporary paper IDs must be supported by other original documents that support the identity.

If you were scheduling for a medical examination, we will be emailing you electronic forms that you will need to complete. Please complete all the required fields and the forms will be automatically transmitted to us.
